Summary:Carcinoma ex pleomorphic adenoma (CXPA) is a rare and aggressive malignancy arising from a benign pleomorphic adenoma. We present what is, to the best of our knowledge, the first case of a primary intrathyroidal carcinoma ex pleomorphic adenoma in an elderly male patient presenting with a thyroid nodule. The differential diagnosis for this lesion is broad and the thyroid nodule should be evaluated promptly with ultrasound and fine needle aspiration (FNA) biopsy based on appropriate criteria as described in the 2015 ATA guidelines for adult patients with thyroid nodules. Given this unique presentation we discuss treatment and anatomic considerations for surgical resection.
