Their trainings are closely related to a hospital’s medical services. There are various types of on-job trainings normally seen in medical settings, and the effects on subsequent clinical performance are far more than predictive in the past. The purpose of this paper is therefore to analyze the correlation between types of on-job trainings and physician’s clinical performance. The study has collected data from 75 attending physicians who have received on-job trainings during the study period in a medical center located in Northern Taiwan. The study period covers from 1996-1998 and 2000-2002, a 6-year duration. The primary findings of this analysis have demonstrated that factors are statistically associated with the clinical performance of a physician after on-job trainings. 1) attending physicians with internal medicine background and received o-job trainings outside the United States; 2) the duration of on-job trainings shorter than one year; 3) the trainings should be in a continuous basis, at better receiving one in every two years. A decline of clinical performance was observed in some physicians received training otherwise; 4) a long-term performance will be connected with a long-term on-job training based on our observations from physicians. For the one of surgeons in this medical center, the best timing to send for on-job trainings was found to be attending year of less than four and larger than eight. In addition, those who received trainings from the USA will have a better clinical performance than that of others. The clues from this study are summarized as follows. Physicians with internal medicine are suggested to send out for areas other than USA. The duration of on-job trainings are better to be a certain period basis, at least one year. The on-job trainings of a surgeon should consider the attending years, that is to select junior ones (<4 atteding years) or senior ones (>8 attending years) while considering the subsequent clinical performance. The trainings are better in areas within USA but duration is not a matter for the clinical performance after trainings. The findings are valuable for reference by policy makers while determining optimal resource allocation and its link with institutional performance. 許光宏 2005 學位論文 ; thesis 87 zh-TW |
