The study framework was mainly using CIPP evaluation model to analyze the literature and interviews. Based on the data, further to develop the questionnaire. The subjects of the study were the professional team workers of 5th disability institutes prize early intervention institutes, including 14 institutes and 34 workers. The study mainly used SPSS for Window/PC (version 10.0) and Microsoft Excel 2000 to analyze the data with frequency distribution、Cochran Q test、posteriori comparisons、and Kuder-Richardson method. The main conclusions of the study were: (1) The service evaluation content included four levels (service plan、service offer、service management, and service efficacy), twelve dimensions(service objective and leader’s views、carry out the service and plan of the propaganda、design the service plan、team build and the way of cooperation、target integrated and elasticity revise、offer relative resources and family services、management of human resources、management and estimation of service record、transit and transfer resources、family’s satisfaction、children’s efficacy, and the use of the evaluation results)and fifty criteria. (2) The KR20 of each level was 0.6627、0.5585、0.5336、0.6950 separately. The KR20 of twelve dimensions was 0.6655, and the KR20 of the fifty criteria was 0.7470. The results indicated that the inner consistence of total scale and dimension scale were good. (3) Referring to the benchmarking practice of American Health Care Criteria for performance excellence, the study recommended the twelve dimensions and the fifty criteria can be integrated into twelve benchmarks.
