Bone mineral density (BMD) is considered as an effective predictor of the risk of osteoporotic fracture. To reduce the risk of osteoporotic fractures, possible risk factors must be taken into account. The study population consisted of 1810 Taiwanese health men (1080 never smokers, 497 current smokers and 233 former smokers; aged 20-80 years) who were measured BMD at the lumbar spine (LSBMD) and femoral neck (FNBMD) by dual energy x-ray absorptiometry (DXA). After adjustment for confounding factor including age, weight, height, physical activity and alcohol intake, current smokers had significantly 3% LSBMD than never smokers (p < 0.0001). Former smokers had lower 3.2% LSBMD than never smokers (p = 0.001), whereas the FNBMD was significantly 3.5% higher than never smokers (p = 0.02) and 2.3% higher than current smokers (p = 0.001). With regard to dose response, heavy smokers (> 15 cigarettes/day) had significantly 3.2% lower than never smokers, but no significant was observed in FNBMD. Additionally, smoking duration was significantly negative in LSBMD (r = -0.129, p < 0.0001) and FNBMD (r = -0.145, p < 0.0001). Moreover, we found that higher 4.1% LSBMD (p) and 2.9% FNBMD (p) in the group of smoking cessation > 5 years than smoking cessation < 5 years. Smoking duration and dose were determent on BMD. Cessations of smoking slow bone loss emphasize the importance of quitting. Duration of cigarette smoking and heavy consumption had deteriorated on bone health.
