As the civilians get sick or injured, just make an emergency call to any emergency medical institution, and the system will be operated immediately to save the patients in most effective period. Taiwan located at the key position of marine trading, and there’re so many vessels cruise around. That’s why marine accidents, seamen sickness and injury events, and any emergency medical cases keep happening. The health problems affect not only patients, but also vessel safety. Therefore, the emergency medical service is as important as the effective capacity of search and rescue (SAR). It’s one of the essential marine policies in Taiwan. Although it’s necessary to establish the maritime EMSS, the regulations and the responsibility range are still new-laid and indefinite in Taiwan. To confer the maritime EMSS in Taiwan is really imperative. We take the EMSS inland as reference in this paper, and it’s composed of three parts including the mechanism, the regulation, and the enforcement. We discuss from the orientation and responsibility of related administrations, laws and regulations, operation procedures, coordination, and human resources training, hoping that will be the reference for building the maritime EMSS in Taiwan. The contain of this research is discussing from the whole structure of “EMSS” , and then gradually focusing on “maritime emergency medical services”. The main idea of this research paper is focusing on the orientation of Coast Guard Administration in the maritime EMSS, vessels dispatching, operation laws and regulations, operation model, medical equipments establishment, and Emergency Medical Technician (EMT) training. The Coast Guard institutions is similar to the Fire Fighting institutions, especial on mission character and orientation, both institution are focusing on “pre-hospital resuscitation”. Therefore, we hope that the specific position of Fire Fighting institutions in EMSS could be the objective in maritime EMSS of Coast Guard institution to strive for. Furthermore, we could use all the medical resources to save all the patients’ lives effectively and under the proportional circumstance. First, we focus on the mechanism. Maritime EMSS is still new-laid, therefore it’s uncertain when it comes to the responsibility to the related institutions and lack of specific regulations, either. “Emergency Medical Services Act” is instituted to stipulate how the EMSS inland operate, and it announced that the Health and Fire Fighting institutions are in charge of the system together. The Act revealed how to coordinate and establish the system. The health institution provides professional medical advices and assistances to train and arrange the emergency resources; the Fire Fighting institution is in charge of operating pre-hospital Emergency Medical Services. After establishing the positions and roles of the related institutions in the system, it would be much effective to build the EMSS. The next, we put emphasis on the regulations of dispatching and operating standard inland and in the air, and then we discuss the possibility to establish the dispatching standard of Emergency Medical Services vessels, other maritime EMS regulations, and Standard Operation Procedure (SOP). After that, we could protect the patients’ and coast guard colleges’ rights and interests to prevent from any medical disputes. The last part is focusing on operating. Comparing the case and the maritime EMSS in these days to foreign maritime EMSS and Taiwan EMSS inland, we can figure out the guidelines and therapies to promote the maritime EMS in operating. After discussing in the mechanism, the regulations, and the enforcement, we provide some advices such as confirming the position of Coast Guard Administration in marine EMSS, establishing EMS vessels dispatching and operation standard procedures or regulations, strengthening the EMS facilities, EMT training on our own, and building the international maritime EMS coordinating structure. We hope that could promote the quality and efficiency of maritime EMS, helping Coast Guard Administration to deal with maritime emergency medical cases.
