Summary:Isolated pancreatic tuberculosis (TB) is extremely uncommon. The combination of its low incidence and nonspecific clinical presentation presents a real diagnostic challenge and complicates management. The diagnosis of pancreatic TB is based on a combination of clinical, biological, and radiological findings, and most importantly, on histological evidence. We report a case of pseudotumoral pancreatic TB revealed by obstructive jaundice in a 48-year-old immunocompetent woman. The diagnosis was confirmed by the presence of small epithelioid and multinucleated giant cell granulomas with central caseous necrosis on biopsies of peripancreatic adenopathy obtained via endoscopic ultrasound. The patient was successfully treated with quadruple anti-tuberculous therapy.
