Summary:Background: Pain medicine in India is gradually gaining recognition as a multidisciplinary specialty. However, establishing a private practice remains a significant challenge. Objective: To highlight key barriers faced by clinicians in setting up independent pain services. Discussion: Regulatory hurdles include licensing of fluoroscopy and ultrasound equipment, compliance with radiation safety and biomedical waste norms, and variable opioid regulations across states. High setup costs, limited trained personnel, and weak referral systems further complicate private practice. Public awareness of interventional pain management remains low, contributing to delayed care and underutilization of services. Conclusion: There is a pressing need for regulatory clarity, interprofessional collaboration, and education of both healthcare providers and the public to support the growth of private pain practices in India.
