Summary:Purpose: to improve results of treatment of patients with the widespread peritonitis, complicated by a macrophage liver failure and endotoxicosis.Materials and methods: this paper is based on the analysis of clinical, instrumental and laboratory examination, medical supervision as well as treatment of 244 patients with disseminated purulent peritonitis (DPR).Results: it was indicated that treatment of the this patients with DPR shall be carried out using a complex of advanced medical technologies, which comprise adequate operational manual, initial empirical antimicrobial therapy, aimed at neutralizing the whole range of pathogenic flora (both aerobic and anaerobic) and intracorporal detoxification (nasointestinal intubation, intestinal lavage, enterosorption, early enteral nutrition, intraportal infusion).Summary: the research revealed that the principal part of fighting endotoxemia and macrophage liver failure belong to measures, preventing translocation of symbiont flora and toxins from the intestine into the portal blood flow and internal environment of the patient
