This synthesis will be generated most effectively if the individuals designing the research and providing the data interact in a variety of ways. Disciplines are isolated from each other and not accustomed to working together; however, this isolation is changing rapidly. Still, there are many issues not yet satisfactorily resolved. This manuscript illustrates these issues and makes suggestions for improving the situation.
