Human error is considered as any activity outside the tolerance limits which are predefined by the system, formally or informally. Besides a review of different approaches and methods for human errors analysis, the Human Factors Analysis and Classification System (HFACS) method will be presented in detail. This relatively new method for the analysis of human errors causes will be illustrated using a real life example. |
