description The subject of this paper is human error as cause of technical and organizational system failure. The risk analysis, as important part of the modern project and processes management, focus increasing attention to the study of human reliability and human error. Human error is considered as any activity outside the tolerance limits which are predefined by the system, formally or informally. Besides a review of different approaches and methods for human errors analysis, the Human Factors Analysis and Classification System (HFACS) method will be presented in detail. This relatively new method for the analysis of human errors causes will be illustrated using a real life example.
