We report an extremely rare case of hypoplastic and duplicated V4 segment of right VA along with variant origin of right Anterior Inferior Cerebellar artery (AICA) from right Posterior Inferior Cerebellar artery (PICA). AICA was duplicated, with one vessel arising from PICA and another arising from distal HVA. Hypoplastic vertebral artery should be considered for the diagnosis of acute stroke of brainstem and cerebellum.
