Algunas reflexiones más sobre la ética de la atención primaria de salud Some more reflections on the ethics of primary health care

En el libro "Temas de Medicina General Integral", editado el pasado año 2001, concluía mi tema afirmando que existen otras muchas cuestiones que deberían ser tratadas, bajo el prisma de la ética, en la Atención Primaria de Salud (APS) y que serían abordadas próximamente en otra publicación dedicada a este nivel de atención. Alertaba, además, que existen problemas serios que pueden convertirse en verdaderos conflictos y engendrar dilemas de carácter ético. También expresaba el criterio de que las Comisiones de Ética Médica constituidas en las instituciones de la APS deberían ser conocidas por todos los trabajadores del área de salud, especialmente su estructura y funciones y las relaciones que pueden y deben desarrollar con el Médico y Enfermera de Familia de los consultorios. Al momento de preparar este artículo, pensé que no era posible dejar de referirme a un valor social que se ha constituido en principio ético universal, la justicia sanitaria, que en Cuba forma parte de la justicia social, la cual, junto con el ideal dignificatorio e independentista, constituye uno de los pilares de la Revolución. Por último, no es posible tratar ningún tema de la APS, sin referirnos a la investigación clínica, epidemiológica y social más importante del Sistema Nacional de Salud que se realiza en este nivel de atención, el Análisis de la Situación de Salud, ni tampoco dejar de considerar la importancia de la participación social y sus implicaciones éticas. El objetivo general de este trabajo es invitar a la reflexión a los profesionales y gerentes que se desempeñan en la APS, sobre estos aspectos que reclaman, tanto o más que ningún otro, no solo el saber y saber hacer, sino, muy especialmente el saber ser.<br>In the book Topics of Comprehensive General Medicine published in 2001, I concluded by asserting that there were many questions that should be approached from the ethical point of view in Primary Health Care and that they will be dealt with in another publication devoted to this health care level. I also warned that there are serious problems that may become real conflicts and generate dilemmas of ethical character, and I expressed the criterion that the Medical Ethics Commissions created at the PHC institutions should be known by all the health area workers, specially their structure and functions and the relations that must be developed with the family physician and nurse of the family physician's office. At the time of writing this article, I considered that I had to refer to a social value that has turned into a universal ethical principle, health justice, that in Cuba is part of social justice, which together with the dignifying and independentist ideal, is one of the milestones of the Revolution. Finally, it is not possible to deal with a PHC topic without making reference to the most important clinical, epidemiological and social investigation of the National Health System carried out at this health care level, the Analysis of the Health Situation. The importance of social participation and its ethical implications should also be taken into account. The general objective of this paper is to make professors and managers working at the PHC level to reflect on these aspects that require, more than any other, not only to know and to know doing, but specially to know being.
