ORTHODONTIC TREATMENT OF ADULT PATIENTS MALOCCLUSION WITH INFLAMMATORY PERIODONTAL DISEASES

Summary:The result of the survey 27 adult patients with malocclusion and periodontal diseases showed an increase of 30-50% of the time of orthodontic treatment. We were treated 58 patients 19 to 55 years, who were divided into two clinical groups. The first came from 31patient with malocclusion, the second made up of 27 patients with malocclusion and periodontal diseases. The survey was conducted on generally accepted methods using clinical and additional (anthropometric, X-Ray) methods. For a comprehensive treatment the following scheme was used: - Therapeutic treatment: included dental health, professional hygiene, skills training in hygiene and patient transfer gum disease in remission; - Orthodontic treatment, which consisted of the use of removable and fixed appliances; - Periodontal treatment - conducting professional oral hygiene during each visit to the orthodontist, vector therapy. The goal of orthodontic treatment was: • removal of plaque accumulation retention points; • redress some teeth to eliminate their functional overload; • stimulation of trophic periodontal tissues; • creation of functional, sustainable, aesthetic occlusion; • improving periodontal condition by stimulating reparative processes in bone tissue; • normalization of function of TMJ and muscles. In some cases, removable orthodontic appliances performed served as of partial dentures. This appliances-prosthetic allowed to carry orthodontic correction allowed to conduct simultaneous replacement dentition defect. Is especially important for adult patients. Activation removable appliances in patients of group 1 was performed 3-5 days, group 2 - every 7-8 days before going to bed, fixed in the 1st group - every 3-4 weeks, in the 2nd - every 6-8 weeks. With that control reviews of patients in group 2 were carried out every 2-4 weeks. As stationary appliances in both groups used ceramic braces or hipoalergenic nickel free metal alloy. Treatment self-ligation a permanent appliances carried out using small forces in the arcs shape memory, starting with .012 ". Changes arcs performed in group 1 was performed every 2-3 months, patients in group 2 not more than 1 time 3-5 months. Duration of treatment in patients of group 1 up 20-22 months in group 2 ranged within 28-33 months. Radiographically in patients of group 1 is only periodontal alteration. In group 2 defined stabilization and suspension of bone resorption. Retention period for the patients of group 1 5-7 years with the use of non-removable retainer in group 2 - retention for life, a combination of fixed and removable retainer retention cap which patients used during sleep. We conducted the treatment of adult patients with malocclusion and periodontal disease (group 2) showed an increase of 30-50% terms orthodontic treatment compared to a group 1 (patients without periodontal inflammation). Clinical observations have shown that orthodontic measures can remove local causes and inflammation in periodontal and cause stabilize the bone resorption and suspension.  Orthodontic treatment appropriate to carry out a two-stage using removable and stationary equipment or its one-step combinations. So, orthodontic part of a comprehensive treatment can eliminate inflammation in periodontal effectively stop the disease process and thereby delay the subsequent development of inflammatory and degenerative manifestations of periodontal and stimulates regeneration.
