There are several published studies on the nitric oxide system, but many aspects of stroke pathogenesis and stroke treatment remain uncleared, there is little evidence about endothelial dysfunction in cerebrovascular pathology. We investigated the role of the nitric oxide system in ischemic stroke pathogenesis in patients with or without metabolic syndrome (MS) and the possibility of treatment of detected disorders. The study enrolled 75 patients with the diagnosis of acute ischemic stroke in the period 2016−2017. The data of ischemic stroke patients were compared to those of 39 absolutely healthy subjects. All patients enrolled in the study were divided into two groups according to the presence or absence of MS, so the group with MS consisted of 36 participants and the control group of 39 patients. It was noted that nitric oxide metabolite values were statistically signifi cant (NO3, NO2) decreased in stroke patients (p<0,001, p<0,001), compared to the group of subjects without stroke, the same phenomenon was also observed for the sum of nitric oxide metabolites (p<0,001) and the NO3/NO2 ratio (p = 0,023). It was found the NO2 and NO3 values (p=0,05, p=0,31), the sum NO2+NO3 and the NO3/NO2 ratio (p=0,48, p=0,37) were not statistically diff erent in the group with MS and without MS. There is a decrease in the level of NO metabolites in the acute phase of stroke, demonstrating endothelial dysfunction, this manifestation being more pronounced in patients with cardiovascular stroke.http://akademos.asm.md/files/48-54.pdfmetabolic syndromestrokenitric oxide metabolites
