Summary:We are reporting an unusual case of actinomycosis which presented to us as a osteolytic lesion in distal femur. Diagnostic confusion leads to delayed medical therapy, increased morbidity and involvement of nearby bone that is, patella. Actinomycosis being an endemic disease and its ability to mimic many skeletal pathology, this should also be included in the differential diagnosis specially when the patient did not fit in to primary diagnosis and respond favorably to treatment given.
