The effectiveness in preventing pregnancy after unprotected intercourse is punctually identified as well as the high rate of preventing pregnancy if used correctly. Numerous studies indicate that Levonorgestrel emergency contraception is not abortifacient. The use of Levonorgestrel administered at a single dose of 1.5 mg within 72 hours of unprotected intercourse is the first alternative; however the Yuzpe method is an important tool if its components are available and should be administered in two doses in first 72 hours after intercourse. Both of them have very similar efficacy, although the second strategy presents more side effects. If time elapsed since intercourse is longer, but less than five days, insertion of copper IUD 380-A, is the first indication. Danazol has been proposed, without reaching approval. New molecules such as Mifepristone, Gestrinone and Ulipristal acetate, with progestogenic properties that make them potentially abortifacient and embryo toxic, have been studied and are in use, although not available in Colombia. The prescription of emergency contraception requires theproper and adequate family planning counseling and specific recommendation that involves the use of a regular method. It is important to demystify the emergency contraception and make it accessible to everyone.RESUMENLos mecanismos de acción de la anticoncepción de emergencia con Levonorgestrel, hoy día son bien conocidos y son claros los efectos sobre la alteración en la ovulación y las acciones periféricas en el moco cervical. La eficacia en la prevención de un embarazo, luego de un coito sin protección, están puntualmente definidos y es alta la tasa de prevención de la gestación, si es correctamente utilizada. Numerosos estudios señalan que el Levonorgestrel dentro de la anticoncepción de emergencia no es abortivo. El uso de la píldora de 1.500 ugs de Levonorgestrel en dosis única, en las primeras 72 horas del coito sin protección es la primera alternativa, no obstante el método de Yuzpe es una herramienta importante si sus componentes están disponibles y debe administrarse en dos dosis en las primeras 72 horas post coitales. Es muy similar la eficacia, aunque superiores los efectos secundarios, con esta segunda estrategia de anticoncepción de emergencia. Si es mayor el tiempo transcurrido desde el coito, pero inferior a cincodías, la inserción del DIU de Cobre 380-A, es la primera indicación. De tiempo atrás el Danazol ha sido propuesto, sin alcanzar aprobación. Nuevas moléculas como el Mifepristone, Gestrinona y Acetato de Ulipristal, con propiedades antiprogestagénicas que los hacen potencialmente abortivos y embrio - tóxicos, han sido estudiadas yestán en uso, aunque no disponibles en Colombia. La prescripción de anticoncepción de emergencia obliga a la adecuada y correcta consejería en planificación familiar y a la recomendación puntual que conlleve al uso de un método regular.
