The historical line of institutionalization of Social Work in its 80 years of existence in Brazil presents to society a profession endowed with a plurality of images and professional identities that vary from coercion and consensus to the profession that fights for social transformation. The research resulting from this production aimed to analyze the relationship between the demands directed to the social worker in the hospital area and the image and the professional identity of the Social Work with a view to pointing out the particularities of the professional work in the high complexity in health. The research was guided by the dialectical-critical method, with a mixed approach, through the technique of documentary analysis to collect data that were interpreted in the light of content analysis (BARDIN, 1979). It was identified that the professional demands directed to the Social Work in the HCPA can be divided into three main groups: i) expressions of inequality experienced by the subjects; ii) measures and services related to the Health Services network, between sectors and at home; iii) patient and / or family follow-up procedures. The systematization of the data and the analysis allow the thesis that the demands sent to the Social Work reflect the image of the profession that is a constituent part of their professional identity, since in their dialectical relation the appearance (the image) reveals aspects of its essence (identity). It can be said that the social worker is the professional demanded to be the link between the users and the institution, the users and the network of services, and also between the institution and the service network. In addition, in his daily work, the social worker has contact with demands, often seemingly simple or displaced from his ethical-political project, but which require a chain of mediations so that provisions and procedures can be leveraged from more social processes from the perspective of guaranteeing rights. === A presente tese sistematiza o estudo realizado sobre a imagem e a identidade profissional do assistente social e a rela??o com as demandas reportadas ? profiss?o no Hospital de Cl?nicas de Porto Alegre (HCPA), l?cus de realiza??o do estudo. A linha hist?rica de institucionaliza??o do Servi?o Social, em seus 80 anos de exist?ncia no Brasil, apresenta ? sociedade uma profiss?o dotada de uma pluralidade de imagens e identidades profissionais que variam da coer??o e do consenso at? a profiss?o que luta pela transforma??o social. A pesquisa da qual resulta essa produ??o objetivou analisar a rela??o existente entre as demandas dirigidas ao assistente social na ?rea hospitalar e a imagem e a identidade profissional do Servi?o Social com vistas a apontar as particularidades do trabalho profissional na alta complexidade em sa?de. A pesquisa realizada orientou-se pelo m?todo dial?tico-cr?tico, com abordagem mista, atrav?s da t?cnica de an?lise documental para a coleta de dados que foram interpretados ? luz da an?lise de conte?do (BARDIN, 1979). Identificou-se que as demandas profissionais direcionadas ao Servi?o Social no HCPA podem ser divididas em tr?s grandes grupos: i) express?es de desigualdade vivenciadas pelos sujeitos; ii) provid?ncias e servi?os relacionados ? rede de Servi?os de Sa?de, entre setores e no domic?lio; iii) procedimentos de acompanhamento ao paciente e/ou fam?lia. A sistematiza??o dos dados e as an?lises permitem que se possa tecer como tese a ideia de que as demandas encaminhadas ao Servi?o Social espelham a imagem da profiss?o que ? parte constitutiva da sua identidade profissional, uma vez que, em sua rela??o dial?tica, a apar?ncia (a imagem) revela aspectos de sua ess?ncia (a identidade). Pode-se afirmar que o assistente social ? o profissional demandado para ser o elo entre os usu?rios e a institui??o, os usu?rios e a rede de servi?os e, ainda, entre a institui??o e a rede de servi?os. Ademais, em seu trabalho cotidiano, o assistente social tem contato com demandas, muitas vezes, aparentemente simples ou deslocadas de seu projeto ?tico-pol?tico, mas que exigem uma cadeia de media??es para que provid?ncias e procedimentos possam ser potencializados a partir de processos sociais mais substantivos na perspectiva da garantia de direitos.
