A experiência do técnico de enfermagem em UTI: aportes psicanalíticos sobre o cuidado e a dor

The Programa Humaniza SUS (Sistema Único de Saúde) has been developed since 2003 and one of its goal is to support the teams which work in the Health Units in some moments of crisis. From the development of some programs of the Federal Government it is necessary to understand not only the demands of the patients but also to aid the work’s team in order to create conditions which promotes the life quality of the professionals in which the caring issue has its aim in the labor practice. Among the different sectors of a hospital, the unit which that critically ill patients are hospitalized who needs the use of technology and who are about to die due to poor clinical condition of these patients is the Intensive Treatment Unit (ITU). This essay intended to explore the emotional condition employed into labor of nursing technicians who work at ITU and are caregivers in their labor daily routine. In order to do that, two sections were written about this topic: a theoretical and an empirical one. On the theoretical part, it is proposed the specificities concerning the issue of taking care of someone, putting as priority the helplessness human condition which is increased facing some illness situation. In order to understand the psychic dynamic which is inherent in this condition, it is going to be used the psychoanalytical theory. The empirical section is going to be done through a qualitative methodology which is useful to investigate some particular aspects of nursing technicians’ experiences in the role of caregivers as health professionals in ITU environment. Some interviews where done which aimed to explore the life history, the motivation of choosing this profession, the aspects that facilitated and also those that complicated in their labor daily routine, as well as the effects that environment and profession have on their intrapsychic field and in the relationship intersubjective between caregiver and their patients. The outcomes of this study were analyzed and discussed through the Analysis of Content. Three categories were identified: Identificatory specificities of caregivers; Effects in the intrapsychic and intersubjective field; Revealing the care action- listening the caregiver. Thus, it was possible to think about the depth concerning the psychic phenomena involved in the task of taking care of someone. Psychoanalysis offered a great contribution to explore and to discuss about the complexity of the factors which are involved in the action of taking care of someone, as well as to think about the exigencies of psychic effort on those emotional demands of the ITU environment. === A preocupação com a qualidade de vida do trabalhador que atua na área da saúde vem sendo colocada em evidência por diversos programas do Ministério da Saúde. O Programa Humaniza SUS (Sistema Único de Saúde) está sendo desenvolvido desde 2003 e tem como um de seus objetivos apoiar as equipes que trabalham nas Unidades de Saúde em momentos de crise. A partir do desenvolvimento de programas do governo federal evidencia-se a necessidade de compreender não só as demandas próprias dos pacientes, mas também de auxiliar a equipe de trabalho no sentido de criar condições que promovam a qualidade de vida dos profissionais que têm no exercício do cuidado o cerne de sua prática laboral. Dentre os diferentes setores de um hospital, a unidade na qual são internados os pacientes graves, que necessitam de utilização de tecnologia e que se encontram em iminente risco de morte devido à precariedade da condição clínica desses pacientes, está a Unidade de Terapia Intensiva (UTI) ou Centro de Tratamento Intensivo (CTI). Este estudo pretendeu explorar as condições emocionais implicadas no exercício laboral de técnicos de enfermagem que atuam em ambiente de UTI e dedicam-se a exercer funções de cuidado como prática de sua rotina profissional. Para tanto, foram elaboradas duas seções acerca dessa temática: uma teórica e outra empírica. Na seção teórica propõe-se uma leitura sobre as especificidades envolvidas no exercício do cuidado, priorizando-se aspectos referentes à condição humana de desamparo incrementada frente à ocorrência do adoecimento. Para explorar a dinâmica psíquica que se faz presente nessa condição, utilizaram-se os aportes da Psicanálise. A seção empírica que também compõem este estudo, mediante a utilização de uma metodologia qualitativa, investiga peculiaridades da experiência de técnicos de enfermagem no trabalho como profissionais da saúde no ambiente de UTI. Foram realizadas entrevistas semidirigidas, nas quais se buscou explorar a história de vida, as motivações da escolha profissional, os aspectos que facilitam e os que dificultam a prática diária, bem como os efeitos que o ambiente e a profissão têm no campo intrapsíquico e nas relações intersubjetivas estabelecidas entre o cuidador e os pacientes. Os achados, neste estudo, foram analisados e discutidos por meio da Análise de Conteúdo. Identificaram-se três categorias finais denominadas: Especificidades identificatórias do sujeito cuidador; Efeitos no campo intrapsíquico e intersubjetivo do cuidador; Desvelando o cuidar – a escuta ao cuidador. Dessa forma, foi possível refletir em profundidade a respeito dos fenômenos psíquicos envolvidos na tarefa de cuidar. A Psicanálise ofereceu uma valorosa contribuição para explorar e problematizar a complexidade dos fatores envolvidos na ação do cuidado, bem como para realizar uma reflexão sobre a exigência de trabalho psíquico imposta frente às demandas emocionais próprias do cenário de uma UTI.
