Biomedical Research Biostatistics Alpha-1 antitrypsin deficiency Lung Transplant FEV1 Alpha-1 antitrypsin deficiency (AATD) allows proteolytic lung damage manifested by decline in FEV1 (forced expiratory volume in first second). Augmentation with AAT may slow FEV1 decline. Infection and rejection after transplantation are associated with higher levels of proteolytic enzymes in AATD. Augmentation is generally not given after transplant. Retrospectively, we compare FEV1% predicted decline of those with AATD (45 [16%]) and AAT-replete chronic obstructive pulmonary disease (COPD) (231 [86%]) stratified by single or double lung transplant. Temporal decomposition was used to plot the trends. Double lung recipients with AATD had a greater FEV1 decline compared AAT-replete COPD. No difference in incident rejection was noted. Multivariable analysis showed higher post-operative FEV1 associated with higher preoperative FEV1, older age, lower Class-I panel reactive antibody, and right lung transplanted.
