The FCU is shown to play an important role in stabilizing the medial elbow during throwing, but there is little research regarding the neural response of this musculature. A measurement called Hoffmann (H) reflex allows for assessment of motor neuron pool excitability (MNPE) of targeted muscles. Previous studies have evaluated effects of joint distention on MNPE in the lower extremity, but there is currently no research on the effects in the upper extremity. Thus, there is no known neurophysiological response of the FCU when the elbow experiences difference in levels of stretch during the act of throwing. Objective: To evaluate differences in MNPE of the FCU using H-reflex when the elbow is placed at increasing levels of stretch (neutral, medium, high). Design: Descriptive. Setting: Laboratory. Participants: 12 healthy males. Inclusion: aged 18-30, recreationally active with history of participation in overhead sport, no history of upper extremity injury or surgery. Exclusion: unable to elicit measurable H-reflex, known central or peripheral nerve pathology, current use of depressants or stimulants, pain with shoulder internal or external rotation. Interventions: Independent variables included level of stretch (neutral, medium, high). All participants completed three measurable trials at each condition. Main Outcome Measures: Maximum H-reflex response, maximum M-wave response, and H:M ratio were recorded and analyzed for observed differences between increasing levels of elbow stretch. Results: Decrease in H-reflex response from neutral to high stretch [ES=-0.63(-1.6,0.4); p=.007]. No difference in M-response [ES=-0.40(-1.5,0.7); p=.520] or H:M ratio [ES=-0.38(-1.4,0.6); p=.700] with increase in stretch. Conclusion: There was a decrease in H-reflex response of the FCU as the level of stretch increased from neutral to high. Level of stretch did not influence H:M response of FCU.
