Summary:博士 === 國立臺灣大學 === 臨床醫學研究所 === 83 === Using immunostaining,immunoblot,reverse-transcriptase polymerase chain reaction and Southern blot,we found that normal urothelium, well to moderately differentiated cell lines and surgical samples expressed E-cadherin and large CD44 isoforms containing exon v6,while poorly differentiated cell lines expressed standard form CD44 and were E-cadherin negative. Sixty-five tumors of one hundred and fifteen transitional cell carcinom examined exhibited a preserved type whilst fifty showed a reduced type for CD44v6. The frequency of reduced type was higher in poorly differentiated carcinomas and tumors with invasive growth pattern than well differentiated carcinomas and tumors with expansile growth,respectively.There was no difference in survival in reference to the expression patterns of CD44v6 and E-cadherin. Among the 113 newly diagnosed breast cancer of infiltrating ductal type,65 express CD44v6. Negative expression of CD44v6 was associated with larger tumor size,more positive axillary lymph node and higher recurrence rate in comparison with CD44v6 positive tumors. Patients with CD44v6 negative tumor had shorter recurrence-free periods after operation. Expression of CD44v6 reduced the risk of recurrence and was an independents prognostic factor as were positive node number and estrogen receptor status by Cox''s multivariate regression. In gastric cancer, the reactivity of CD44s and CD44v6 antibodies did not link to histopathological and clinical prognostic factors in intestinal-type carcinoma. In contrast,expression of CD44v6 was associated with infiltrative tumor growth,depth of invasion, lymph node involvement and higher incidence of presentation with distant metastasis in cancer of diffuse type. This study showed the role of CD44v6 in invasion and metastasis of diffuse-type gastric carcinoma and demonstrated the necessity of sub classifying tumors in study significance of CD44 in human cancer.
