It was a two-stage study. In stage I, in-depth interview was conducted to explore the contents of uncertainty an AMI victim experienced in intensive care unit (ICU), in ordinary ward, and prior to their discharge. Three subjects from a regional hospital in Hualien were enrolled and the interviewed contents were processed by content analysis techniques. The uncertainty items concluded from content analysis were used to develop a rating scale which was used in the second stage of the study. Thirty AMI patients admitted to a medical center in Taipei city during the Spring of 1997 were enrolled in second stage of the study. Data were obtained by interview guided by the uncertainty rating scale. The major statistical procedures applied in data management were: frequencies, t-test and ANOVA. The results indicated that patients experienced uncertainty about the disease, their prognosis, and the treatment/diagnostic procedures during their hospital stay. However, the foci of uncertainty varied in different periods. Uncertainty specific to period of ICU stay was related to family adjustment. Uncertainty specific to the period of ordinary ward stay was regard to the treatment/diagnostic procedures and management plan. Uncertainty specific to the time prior to discharge was related to the weakness of physical status and self-care at home to handle the possible recurrent of the disease. Patients with age less than 65y/o, single, currently owned a job, receiving high school education or less, new to AMI, and receiving angioplasty, stent, or thrombolytic therapy had high level of uncertainty during their stay in ICU and ordinary ward (p<.05). At the time prior to discharge, female patients, with left heart infarction, or ever heard of AMI before had higher level of uncertainty (p<.05). The findings of this study provided references to identify AMI patients who tend to experience uncertainty and the contents of uncertainty a patients might be. It was suggested to assess and recognize patients' uncertainty in providing care.
