This technique is introduced to a control system by evaluating failure modes and causes of failure to predict and estimate potential failure in every subsystem. By analyzing reliability block diagrams, suitable actions can be taken in advance to minimize the negative effects. This paper intends to expand the application of FMEA to the equipment maintenance system of a manufacturing factory. By implementing this technique, potential failure modes and effects on facilities can be estimated. This information may help management prioritizing the corrective actions to eliminate or prevent the occurrence of failure. The equipment maintenance activities can then be appropriately conducted, leading to the timely delivery of order, better quality of product, low operational cost, enhanced safety of employee and etc..
