Summary:碩士 === 國立臺灣大學 === 醫療機構管理研究所 === 89 === The Purpose of this study is to explore rural-urban differences for the functions between primary health centers in Taipei County. The author conducted a cross-sectional telephone interview survey in the 29 towns in Taipei County. The sampling method is probability to proportion random sampling. The telephone interview is through the Computer-Assisted Telephone Interviewing System(FJS-CATI), and sample size is 8033. The major findings are: (1) 16.5% of the people had used the services of primary health centers in the pass one year. (2) Satisfaction and utilization are consistently that country areas are higher than the semi-city areas, and semi-city areas are higher than the city areas. (3) The health need is different in rural and urban areas. Country areas need medical care services, but semi-city areas and city area need more preventive medicine. (4) The logistic-regression results show that, the factors of education status and living in city areas have a negative effects on the utilization, satisfaction, and importance of primary health centers in Taipei County. According to the results of this study, four suggestions are proposed as follows: (1) Group the same areas, such as country side areas together primary health centers and establish a board-health-center to strategic alliances. (2) Re-allocate health resources according to this new grouping, and modify the function of the primary health centers according to the group. Country areas must pay attention to the medical and health prevention. But, city areas pay attention to health prevention. (3) Encourage medical suppliers to establish hospitals and increase the medical access in areas with insufficient health care resources. (4) Modify the division of current medical network and sub-network.
