Summary:碩士 === 國立高雄醫學大學 === 公共衛生學研究所碩士在職專班 === 90 === ABSTRACT Research Purpose: To save the National Health Insurance Bureau medical care costs to diminish financial resource difficulties if Acetaminophen, a prescribed medication, can be offered at the lowest medication price of health insurance or at the insurant''s liability for billing balance when in or above the regional hospitals in Kao-Ping area submit the application for health insurance medication expenses. Research Method: To calculate actual Acetaminophen oral users of 500 mg single formula against actual budget requested for the last quarters in 1999 and 2000 at 3(three) medical centers and 8(eight) regional hospitals of Kao-Ping area. The statistics methodology is to analyze and study with a complete set of SPSS. 10.0 statistics software. Research Result: Due to differences in prescription patterns, the medication quantity and cost amount aforementioned hospitals are influenced by their hospital size of and number of diagnosis subjects. It has been disclosed that these hospitals prefer to adopt higher cost at NT$ 0.74 ~ NT$ 1.00 per Acetaminophen tablet per prescription. To analyze as per the nature of authority, there is no apparent difference in average prices between civil and public hospitals. However if the prescription is analyzed per rating degree, the price is way higher in medical centers than that in regional hospitals. Conclusion: At present, the health insurance medication is being paid by the third party for "Fee-For-Service." Medical providers and insurants don''t seem to have any economic sense, and that makes the enhancement for medication cost necessary year after year. The National Health Insurance Bureau should commence to enforce the preliminary health Insurance regulations for prescribed medications to eliminated budget, or to consider it in reference to "Reference Price" that is being practiced in Canada to avoid improper use or waste of medical resources.
