Dentists’ Strategy Adoption vs. Global Budgeting Payment System

碩士 === 國立成功大學 === 企業管理學系碩博士班 === 90 === Abstract In order to control the growth rate of health expenditures, the Bureau of National Health Insurance（BNHI） has applied the Global Budget Payment System on dental service as of July 1st 1998. This study employs the medical payment data from Southern Branch of BNHI to analyze how The Global Budget Payment System has influenced physician behaviors. In the meanwhile, the dentists’ Strategy Adoption in response to the new system is investigated. We found： （1）Physician behaviors have been influenced by the Global Budget Payment System. Dentists have changed the quantity of dental service items. Patient visits, the amount of the service cases, and the total points of the medical service have changed. （2）After implementing the Global Budget Payment System, there are 30.8 percents dentists have reduced medical service items. There are 34 percents dentists have increased medical service items, 36.2 percents dentists remain the same medical service items. Two third dentists have reduced the amount of services cases, but one-third dentists increased. （3）Dentists are classified according to adoption of strategies, and named ”positive defender”, ”negative defender”, ”positive reactor”, ”negative reactor”. According to the research, positive defender is the youngest（40 years）and negative reactor is the oldest（45 years）; Most of the positive defenders（68.6%）practice in urban area. （4）As to medical yield, positive reactor and positive defender yield more than negative reactor and negative defender.
