Name of Institute: Graduate Pages: 178 Institute of Guidance and Counseling Graduation Time: 93/06 Degree Conferred: Master Student Name: Shan-Cheng Ko Advisor Name: Prof. Wei-Su Hsu Prof. Wen Hsiao Abstract Using as a theoretical frame─Geertz’s theories that see as a text the life story and dialogues of mentally challenged people and their psychiatrists, and the culture they are in, and Foucault’s theories on history, discourse, and power/knowledge, this research aims to provide a different perspective from which mentally challenged people shall be seen, by investigating the conspiracy of doctors and patients, which includes such issues as the history of psychiatric treatment, de-institutionalization, the double authority of medical institutes, professionalism, medicalization, doctor-patient relationships, and social stigma branded on mentally challenged people. Mentally challenged people’s malfunction and maladjusted actions are prone to be ill-characterized in the context of securing power in the culture. With the medicalization of social problems, the whole culture conspires to form a medical discourse that produces a mechanism for a concentric power. Some doctors believing such a mechanism are thoroughly convinced of their professionalism and ignore the limitations brought on the autonomy of their clinical performance by policies formulated by medical institutes and the National Health Insurance Bureau. Furthermore, mentally challenged people and their family members and relatives face a double bondage or a dilemma caused by this mechanism when they consult a doctor and try to learn about more medical knowledge of mentally illness and proper medication. Therefore, in order to improve the medical treatment of mentally challenged people, the psychiatric treatment team should not only regard mentally challenged patients as customers, but also should help the public to better understand mental illness and to change their vicious attitude toward it. Mentally challenged patients should be encouraged to form enthusiastic participation and autonomy during treatment and rehabilitation, to build a good relationship with their psychiatrists, and to re-trust themselves and the world as well. Moreover, the government’s positive education and the media’s reports on mental illness also play an essential role in establishing more rehabilitation organizations in communities, widening the public’s acceptance of mentally challenged people, and thus creating a more friendly environment for doctors and patients in their collaborative conspiracy.
