Summary:碩士 === 國立台北護理學院 === 護理研究所 === 92 === The purpose of this study was to develop a case management program for patients with Chronic Obstructive Pulmonary Disease(COPD), and to test its effects on the length of stay, medical cost, health-relative quality of life, satisfaction of the patients and medical personnel. The study is a quasi-experimental design. Subjects are patients diagnostic COPD with secondary infection, medical and nursing staff. The study was carried out in a medical ward of a hospital in the northern Taiwan. Data were collected from the control group of 24 patients first, then the experimental group of 18 patients. The first stage of data collection was from September of 2003 to January of 2004, and the Second stage was from March of 2004 to June. The research instruments included basic medical record charts, Health-Relative Quality of Life measurement, COPD Knowledge Measurement scale, Patient Satisfaction with Nursing Care Questionnaire, and Six-Dimension Scale of Nursing Performance. Control group patients were cared with unit routine. Experimental group patients received care as indicated within COPD case management. The software SPSS10.0 used for data analysis. The statistical methods included: frequency, mean, standard deviation, percentile, chi-square test, independent sample t-test,pair t-test. The results indicated that case management is effective for chronic obstructive pulmonary disease patients. There was no significant difference in length of stay, medical cost and Health- Relative Quality of Life. However the case management improved the disease knowledge, satisfaction of patients and the job satisfaction of nursing staff. This study supports that case management is a useful medical management program to reduce the length of stay and medical cost. Furthermore, it also improves the knowledge, satisfaction of patients and the job satisfaction of nursing staff.
