The study was a prospective cross-sectional and correlational design. One hundred and twenty patients who met the criteria for type 2 diabetic patients were recruited from a medical center in northern Taiwan. The research instruments included Insulin Management Diabetics Self-Efficacy Scale, Diabetics Self-Care Scale、Adult Diabetics Dependence Quality of Life. Data were analyzed with mean, standard deviation, range, frequency, percentage, Pearson’s correlation, and linear regression by using the SPSS statistical package for windows version 10.0. Data analyses revealed that self-efficacy has a significant positive relationship with self-care (r = .80,p = .000) and has a significant negative relationship with glycemic control (r = -.64,p = .000), A significant negative relationship between self-Care and glycemic control was noted (r = -.77,p = .000). Furthermore, a significant positive relationship was found between quality of life and self-efficacy (r = .22,p = .014) or self-care (r = .28,p = .002). The findings of the path analysis showed that self-efficacy and self care have a positive and direct relationship (b = .80,p =.000), self- care and glycemic control have a negative and direct relationship (b = -.71,p = .000), self-care and quality of life have a positive and direct relationship (b = .43,p = .09). An indirect effect of self-efficacy on glycemic control mediate through self-care was found (.80 * -.71 = -.56). Self-efficacy through adherence to self-care could contribute to control glycemic total effect (b = -.50), Self-efficacy has no effect on glycemic control (p> .05), glycemic control was also not related to quality of life (p> .05). This research utilized the path analysis model to investigate the direct and indirect relationships between the self-efficacy and self-care or glycemic control or quality of life. On the basis of the result that we hope the clinical nurses should address to promote the type 2 diabetic patients’ self-care, which would improve the quality of life.
