The following influencing variables were selected based on Andersen’s service-use model: demographics, social structural factors, family resources, and felt and assessed needs. This study is a cross-sectional survey using purpose-sampling method from January 1, 2006 to May, 31, 2006. Using the questionnaire, discharge planning nurse in one central Taiwan regional hospital interviewed patients and their carers. Totally, 142 patients were interviewed. The result shows that, of all the participants, 31.9% entered the institutional long-term care facilities. Multiple logistic regression analysis showed that education, family resources, charge for entering the facilities, self-rated health, chronic comorbidities, be catastrophic were significantly related to entering the institutional long-term care facilities after discharge. “Aging in Place” is the guiding principle for devising elderly care policy in Taiwan. However, with the rapid and dramatic change in population composition, social change, and westernization of Taiwan, moving into the institutional long-term care facilities is inevitable. To some extent, aging in place may delay the time for entering institutional long-term care facilities, but can not replace it completely. Consequently, a more comprehensive transfer system among patients, hospitals, and long-term care facilities is needed. Furthermore, it is imperative for policy makers to have more considerations to access, quality, and cost, when developing the elderly care policy, to assure that all citizens receive a level of health care service adequate for their fundamental needs. 梁亞文 2006 學位論文 ; thesis 113 zh-TW
