It adopted a cross-sectional research design and purposive sampling in 6 nursing homes. The total number of staff was 74, resident was 269, research tools included general self-efficacy, knowledge of restraint and attitude toward restraint use questionnaire, data was analyzed by SPSS12.0 statistic software. This study explored that staffs’ attitude toward restraint use and physical restraint ratio was different between upper grade and under grade nursing homes. The prevalence of physical restraint in nursing home was 48.0%, the main reason of use restraint was to prevent fall. Residents’ Bethel index score, SPMSQ score and fall history were predicted factors of physical restraint. There were limitations in this study which were general self-efficacy scale and knowledge of restraint scale didn’t recheck reliability and validity, date of observation are not radon assignment which may lead the result can not be generalized to population. According to research the result, researcher suggested that staffs need advance their physical restraint knowledge and ability of use restraint alternative intervention in practice; nursing home managers should made physical restraint process, provide a safe environment, give correctly restraint knowledge and alternative intervention skill to staffs for reducing the use of physical restraint in nursing home.
