On February 5, 1999, the Tamshui Branch Hospital of Mackay Memorial Hospital established Taiwan’s first Hospice and Palliative Care Center. This 18-beds hospice center not only promoted Taiwan to the world symbolically, but also had Taiwan to become the 18th country to set up hospice institute. That day also represented a milestone on the history of hospice in Taiwan, and initiated its later vigorous development. The purpose of this study is to understand the ecological environment of the hospital, proposing suggestions for the future development by predicting the supply and demand in the next twenty years headed on the present situation of hospice and the quality of its sickrooms it needs, and by probing the future tendency in the development of the hospice from the perspectives of the experts and the staffs. According to the prediction of the Council for Economic Planning and Development (CEPD), Taiwan has nearly two millions older people now and by the year of 2010, half of them will not be able to look after themselves and need personal long term care. Therefore, the government should plan to sustain the public responsibility of researching on disease for prevention and treatment and educate people how to deal with and take care of the older people through the system of community and school. The methodology applied here in this study is Qualitative Inorganic Analysis, probing the situation by using Discovery-oriented Process through the interviews to understand the direction of future development. The way of interview was in accordance with Delphi Method and the core cluster is the doctors who are in charge of hospice care and hospice clinic and research on the cancer diseases. Together with the working experiences of the clusters such as pastors, senior nurses, social workers and volunteer workers of the hospice center, the analysis attempt to speculate about the situation in the future. The result of this study shows that the present hospice system is in a stable condition and also could be concerned internationally, and thus the government should establish a special unit to watch over it. However, because of the growing population of older people, the immature environment for long term care, and the problem of insurance policy, the present hospice system should learn from and discuss with associated foreign organizations or institutes more in order to develop a hospice pattern of long term care combined with the communities. The increasing of service quality and hardware quantity of hospice is about to predict in the coming twenty years. Hospice is a necessary system during the transformation of double income families in the family structures in the society. No matter how long will it take, people should have a right attitude toward the issue of death and the educational knowledge it bears. That is to say, people should comprehend the meaning of life and death, and hence be well-prepared to help themselves or their relatives to take their last sleeps. Finally, assistances with the clinic experiments of experts and doctors, people could propose their advises to the government objectively and force the systems such as the government, the hospital and the school to consider the concept of hospice seriously, and then make laws to put it into practice in order to let the philosophy of caring before one’s death can deeply root in the heart of everyone of us.
