The participants of this study are 854 General Practitioners who are also members from the “National Health Insurance Bureau Contracted Clinics” in the Yu-Lin and Changhua County. The survey was conducted by mailing out research questionnaires, of which a total of 234 valid responses were returned, about 27.49%. The research was compiled using a SPSS10.0 question and answer format.
The following descriptive statistical analysis are found: (1) 83.3% of General Practitioners wish to exclude the budget allowed in the trial from western medicine budget total. The amount of funds received for this trial and the guaranteed income for the family physicians are the main concern for General Practitioners. (2) Those who provided positive feedback to the trial are skeptical whether the Family Physician Care system fits in the present medical system in Taiwan. About 50.4% of the General Practitioners feel that the Family Physicians Care system in Taiwan cannot achieve the goal of differentiating levels of care and referral. (3) Most of the General Practitioners disagree with the Family Physician Care quality standard index set by National Insurance Bureau. (4) About 65.4% of General Practitioners that completed the Family Physician licensing training program feel inadequate to practice family physician care. (5) Most General Physicians feel the general public lacks proper guidance and the program is not adequately promoted by the government (6) In addition, the public are used to the freedom of choosing any type of doctors and medical care. This creates an obstacle to regulate the public to follow the rules under family physician care system in which the members can only pick the doctors who are within the system network. It also increases the difficulty to manage the members within the system.
The results are as follows: the budget for this plan should be excluded from the total amount of budget (P<0.05); family physicians should be provided with medical care fund (P<0.001); family physicians clinics should receive assurance (P<0.05); the insurance expenses for the family physician care system members should be decreased (P<0.05), the promotion of the family physician care system needs to be promoted aggressively (P<0.001); the issue of whether the family physician care system fits the present medical system in Taiwan needs to be further investigated (P<0.05); general practitioners should participate in the family physician care system (P<0.01); family physicians care system needs to work on achieving the goal of differentiate levels of care and referral (P<0.01); family physician care system quality care standard index needs to be improved (P<0.05, P<0.01, P<0.001); Family physician care system implementation fund aid (P=0.000) and execute family physician care system members managements (P=0.000) are all relative.
Conclusion:
1. Independent special fund source should be set up for “ The Family Physician Care Trial Plan“.
2. Family doctors should be forced to receive continued clinical new information training with specialized program for each area. Award plan should also be set up to increase the participating rate.
3. Increase the awareness and medical training.
4. Educate the general population about the important role of family doctor. Award plan should also be designed for patient who visited the family doctor first instead of going to medical center directly.
5. Execute the designed system plan which the family doctor should take good care of local community.
