In common use、the most frequent treatment to cure patient is the medicine therapy. Therefore medication error is the key point to influence the medication safety. It’s also an important assignment to medical management. The purpose of this study is to discuss doctors and pharmacists』 opinions toward the cognition of medication error and management of medication safety.
The research in this study is at a teaching hospital in Hsin-Chu area. According to Gennaro(2000)’s definition toward medication error and the factors that cause medication error we collected in practical hospital during last year、We classify the factors into different types which is the foundation of the questionnaire. We begin using the study by questionnaire、the subjects of this research are doctors and pharmacists in the practical hospital. We use common survey for this investigation. Doctor’s questionnaires are 84 pieces and we received 60 of those. The effective receiving rate is 71.42%; Pharmacists』 questionnaires are 32 pieces、and we received all of them. The effective receiving rate is 100%
This survey find that in pharmacist’s opinion、the main factors of the medication error are「the similar name」、「the similar looks」、「dosage error」and「patient took the wrong medicine」。In the doctor’s opinion、the main factors of the medication error are「Doctors don’t decrease the dosage when patients』 liver and kidney’s function are weak」、「reciprocal effect causes the dosage lower or higher」、「medicine reciprocal effect causes the harmful reaction」and「Not to ask the patients about medicines over-sensitive」。 。
