An Exploratory Study of Hospital Medical Service outsourcing Management–Using Hemodialysis as an Example

碩士 === 中國醫藥大學 === 醫務管理學研究所 === 96 === Objectives: This study aimed to explore medical service outsourcing decision-making and operations management of hemodialysis in hospitals. Outsourcing contracter selection, supervision management, and outsourcing performance assessment are three major issues. From analyzing of hemodialysis outsourcing services, we hope to provide hospital specific recommendations for medical outsourcing management. Methods: This study applied both qualitative and quantitative methods. It includes in-depth interviews, document analysis and questionnaires survey. In-depth interviews adopt snowball sampling tochnigue in purposeful sampling, using a semi-structured questionnaire to survey hospitals and contractors. To conduct document analysis, we collect medical service contracts and selection specifications of hemodialysis from public hospitals. This studay also conducted questionnaire survey of thirty hospitals with medical outsourcing hemodialysis. Results: The study found that financial feedback arrangement out-weights other factors as the major criterion in contractor selection by hospital. On the other hand, outsourcing contracter，s major concern is hospital’s interity. It was found that politics still play a role in outsourcing decision-making for public hospitals. The major reason public hospital adopt outsourcing is because contractor has marketing expertise and flexibility in operations arrangement. Overall 60 percent of the hospitals with outsourcing medical service had set up outsourcing performance committee. Around 80 persent of the hospitals are satisfied with its outsourcing service performance. Hospital ownership, size, and leaders with management master degree are not significant regarding outsourcing service performance. Conclusions and Suggestions: The study recommends hospitals select physician lead contracter as better outsourcing choice. The selection criterion should include operating plan, project management plan and contract renewal assessment. Hospital should establish comperhensive selection mechanism, outsourcing contract management and renewal procedure to enhance the quality of medical outsourcing management. Hospital should also set up outsourcing management committee to monitor contract implementation and could provide detail information regarding contract renewal decision making.
