Summary:碩士 === 中原大學 === 國際貿易研究所 === 96 === (Ⅰ)Purposes: Taiwan's male receives tradition “the patriarchal clan principle” consciousness state influence, hoped that reaches the goal of generation to generation because of marriage. But Taiwan’s female selects someone to marriage has the marriage slope, in addition the Economic independence makes the phenomenon of unmarried to be obvious. The social underlying male in domestic is not so easy to find someone to marriage, had to seek overseas. For nearly 20 years, took a phenomenon of marriage immigration, the special marriage motivations of them, make they face strong stress of immediately pregnancy after marriage. The immigrant spouses utilize the initial timing and the number of prenatal care visits affect themselves and fetal health. Previous studies, mostly used by a questionnaire, face visits manner. The Department of health, published the rate of prenatal testing for 10 times as the estimate times. In this research we attempts using three kinds of measuring index to compare. We test whether immigrant spouses made the timing of initial prenatal testing in the first trimester, and then find the most important influenced factors involved. (Ⅱ)Methods: The data was collected from National Health Insurance from 2003 to 2007, which participated in NHI in 2008 and had ever been parturition, the total study population is 14,028. In order to understand the differences among immigrant spouses background information, and then analyzed their influence factors of prenatal testing by using χ2, Independent-T test, One-way ANOVA and logistic regression. Finally the total number of prenatal testing will be divided by the continual style, the dichotomy, and the quarter then converged the Eviews software, we use the Integer Count data, Binary choice-Logit, and Ordered choice-Probit to compares the difference among three kinds of model. (Ⅲ)Results: This study discovery age and society is the major effect factors of prenatal testing. With low-income households, less than 20 years old of immigrant spouses prenatal testing is inadequacy. But immigrant spouses made an adequacy prenatal testing in private hospitals or primary clinics. However, the immigrant spouses’ parturition in the Miaoli county, public hospitals, take care of by less than 30 year old male physician, who made prenatal testing, is inadequacy. Besides, infant birth weight was less than 2,500 grams made higher medical expense. In addition, over 50% immigrant spouses made initial prenatal testing was not in the first trimester, and immigrant spouse’s parturient low-weight infants had an inadequacy prenatal testing. (Ⅳ) Discussions: Immigrant spouses in own backgrounds, prenatal characteristics, medical institutions, and the utilization of prenatal testing be different. This study found that more than half of immigrant spouses whose initial prenatal testing is not in the first trimester, but people pay attention to the number of prenatal care visits(10 times s the estimate times). The initial timing of prenatal testing is not so much be attention. It is recommended that government can consider adopt prenatal testing index from abroad to measurement. “Pay-for-Performance” may be a good incentive to improve immigrant spouse’s initial visit in the first trimester. It may be an effective strategy that from top to bottom comprehensive coordination situation, to enhance immigrant spouses attained qualified prenatal testing to prompt maternal and infant’s health.
