Summary:碩士 === 國立成功大學 === 高階管理碩士在職專班 === 96 === The rationale of this thesis is to investigate the special needs of Taiwan medical ecology from the global background, focused on the potential structural problems of care system. The purpose of this study is to reform the medical system with the specific aims to improve the medical education and physician attributes and to innovate the infrastructure of the care system and national health insurance policy. While with the understanding of gaps between the idealistic and practical sides, it is expected to incorporate two core aspects of medical system into reforming process, including the managerial aspect that would be practical with consideration of idealism and the social responsibility aspect that should be idealistic with practical consideration. It is a belief that the cogent analysis of problem will lead to ways of solution. The thesis should be categorized as a qualitative analysis with four hypothesis and consequently four themes. Observation, summarization, case study, literature and clinical reasons are used to substantiate the following statements: 1.The medical doctors should be fully devoted to the sanctity of life with the spirits of altruism, in order to reach self-realization. Nonetheless the medical career might be affected by the medical ecology and national health insurance system with inevitable dilemmas between managerial economics and social responsibility aspects. 2.Reforming the medical system of Taiwan in the past few years based on financial management but the negative impact on the hospitals and physicians, especially concerning the social responsibilities should be evaluated.. 3.Both the problems of medical system and the barriers of reformation originated from the special background and specific competition. 4.Reforming with the purpose to improve the medical education in the past few years had been focused on the social responsibilities, humanity and ethics, but the reality and difficulties generated by the medical system on the way to reach such a goal should be further investigated. It is believed that only when the above problems were specified and the solutions were identified could the medical ecology be improved through education and national health system be promoted through reformation for the better farewell of people in Taiwan.
