In Taiwan almost all areca quid chewers are also smokers, and whether areca quid and smoking will generate higher oxidative stress has not been studied. This study compared the urinary levels of oxidative stress markers 8-hydroxy 2'-deoxyguanosine (8-OHdG), 8-iso Prostaglandin F2α (8-iso PGF2α) and 7-methylguanine (N7-MeG) in subjects with known areca quid and smoking history. A total of 134 normal subjects were recruited and their urinary 8-OHdG, 8-iso PGF2α, and N7-MeG levels were quantitated using liquid chromatography tandem mass spectrometry (LC/MS/MS). The results demonstrated that when each subject was divided into smoking and/or areca chewing habits, smoking and chewing habits alone generated higher urinary N7-MeG and 8-iso PGF2α levels (4862.56, 5165.05, 5080.82 vs. 2873.02 ng / mg Cr; 0.171, 0.170, 0.164 vs. 0.114 ng / mg Cr). On the other hand, urinary 8-OHdG levels were not different in smokers and areca quid chewers. The smoking and chewing group generated lower urinary N7-MeG and 8-OHdG levels than only smoking group, but not significant (4862.56 vs. 5165.05 ng / mg Cr; 4.45 vs. 4.88 ng / mg Cr). In conclusion, this is the first study to point out that chewing areca quid generates oxidative damage to lipids, as evidenced by elevated urinary 8-iso PGF2α levels. Consequently, 8-iso PGF2α may serve as a valuable and sensitive indicator of oxidative stress for areca quid and cigarette exposure. These results suggest that areca quid inhibits cigarette induced oxidative damage maybe due to polyphenols.
