The bone quality of calcaneal was measured by quantitative ultrasound (QUS). Anthropometric data and reproductive history were collected from questionnaire. Two hurdred and forty-third patients with hemodialysis (aged 26-94 years, mean age of 64.4 ± 12.8 years, duration of dialysis 75.18 ± 60.7 month) and 256 normal controls (aged 46-85 years, mean age of 63.6 ± 9.3 years). The results indicated that speed of sound (SOS) decreased 12.2 m/s, broadband ultrasound attenuation (BUA) reduced 6.3 dB/MHz, and quantitative ultrasound index (QUI) loss 7.9 % in hemodialysis patients compard with normal controls. Morever, BMD and ultrasound parameters was higher with BMI>24 kg/m2 and 100-300 pg/mL of PTH level, the statistically significant did not achieve (p>0.05). BMD and ultrasound parameters was significant higher among moderate exercise (p<0.05). With regard to biochemical analysis, alkaline phosphatase and Ca×P were negatively associated with BUA(r=-0.17, r=-0.16, respectively). Calcium was positively associated with BMD, SOS, BUA and QUI(r=0.17, r=0.15, r=0.18, r=0.17, respectively). Phosphorous was positively associated with BMD, BUA and QUI (r=0.16, r=0.16, r=0.15 respectively). This study demonstrated that hemodialysis patients had obvious bone loss than normal controls. By using QUS and biochemical analysis of bone turnover. In conclusion, regular monitored bone quality in hemodialysis patients were essential to prevent the osteoporosis.
