The reason for developing the system was that the manual method was complicate and not easy to manage. Meanwhile, information about residents’ leisure activity service was required to meet the accreditation by the Department of Health. Therefore, this study based on the current problems of the case nursing home, related literature reviews, accreditation standards, expectations from residents’ families, and design of Information system engineer, to build a prototyping of “activity records information management system”. The development process was divided into two phases: the first phase was to collect requirements in order to develop the system and the second phase was effectiveness evaluation after the system was developed. The system was evaluated from five parts: the value, interface, service platform, organizing model, and revenue/cost through direct interviews of 8 system users and 2 family members. The results showed that: (1) the value of the system: users held positive feedback on the “activity records information management system”. Family members indicated that the output of the system had significant meanings to the residents families; (2) system’s interface: users were satisfied with the content, (3) service platform: the recorded information could be used for better understanding of clients by new staff members or students which also fulfilled the accreditation requirement; (4) organizational model: users believed that the system helped collect information not only meeting the needs of the service providers but also assisting transitions among group leaders; (5) revenue/cost: since there was no charge for participants, therefore there was no revenue yet. However, users were confident on the saving of labor cost and would recommend the system to other institutions..
