Ballistic goal directed movements are controlled by triphasic activation pattern of agonist and antagonist muscle pairs which was proved to be generated by pre-movement facilitation in motor cortex. Disrupted pre-movement MEP facilitation pattern and disrupted triphasic EMG pattern were found in patients with spino-cerebellar ataxias (SCA) who had hypermetria. Previous studies have shown that motor cortex excitability could be modulated by peripheral ES and motor training. The purpose of this study is to find out whether the temporal pattern of antagonist muscles pre-movement facilitation can be influenced by temporal ES assisted training
in healthy and SCA subjects.
Methods: 20 healthy and 20 SCA subjects were all randomly assigned to intervention and control group. Intervention group in healthy subjects received 30 mins delay antagonist activation temporal ES. The intervention group in SCA subjects received four weeks temporal ES assisted home training program. The pre-movement MEP of FCR (antagonist), EMG of FCR and ECR(agonist) and movement error during fast 30
degrees wrist extensions were recorded before and after interventions.
Results: Abnormal antagonist pre-movement facilitation temporal pattern, longer antagonist delay onset time and greater movement error were observed in SCA. In healthy subjects, the antagonist pre-movement facilitation pattern, antagonist delay onset time and movement accuracy were changed after delay antagonist activation temporal ES intervention in healthy subjects. In SCA subjects, after received four weeks of temporal ES assisted home training, improve the antagonist pre-movement facilitation temporal pattern was restored, antagonist delay onset time and movement
error were improved significantly.
Conclusion: Peripheral temporal ES can influence the temporal pattern of pre-movement facilitation. Four weeks of temporal ES assisted training can restore temporal pattern of antagonist pre-movement facilitation, shorten the antagonist delay onset time and improve the movement accuracy of ballistic goal directed movement
for individuals with SCA.
