In recent years, the Bureau of National Health Insurance (BNHI) has been focusing on ensuring care for socially and economically disadvantaged people, and steadily improving its administrative efficiency and service quality. Today, Taiwan's National Health Insurance (NHI) system has become a role model of a single-payer social insurance and has earned praise in the international society.    With the introduction of a global budgeting system, which sets annual spending caps on broad health care sectors, the BNHI must ensure that health care quality in Taiwan will not be compromised due to resource constraints. The Bureau has consistently worked together with the medical institutions to provide quality care that often time goes beyond the call of duty to better satisfy the needs of the insured.    The Bureau has drafted strategic goals and concrete measures that are expected to enhance healthcare functions at every level of the system and strengthen community care. on January 4, 2011, the NHI past (called the Second-Generation NHI)the third reading in the Legislative Yuan and became law. on January 26, 2011 the NHI program was launched.    This research with the historical Institutionalism analyze the development of National Health Insurance,. In Global Budget System adoption and tiny view , is carried out effort fee-for-service payment system to DRG payment system. And the amended National Health Insurance Act was promulgated by the President on January 26, 2011, signifying an important reform of the first-generation NHI program which has been in implementation for 16 years. The purpose of the amendment is to ensure greater fairness in the premium rate, basis for premium calculation, and methodologies for determining premiums. It is anticipated that the 2nd Generation National Health Insurance Reform can help expand the funding base for the National Health Insurance system and making the sharing of the financial burden imposed by the system fairer and more equitable. Furthermore, the amendment will also serve to safeguard the rights and interests of the NHI participants in receiving medical care, protect the health of the population, and improve the quality of medical services.
