碩士 === 國立陽明大學 === 物理治療暨輔助科技學系 === 100 === Background: The workers with occupational accidents (OA) are lead to disabled and have problem in job and financial. There were plenty of problems for return to work in the workers with OA. Study show programs with vocational rehabilitation enhance the process for workers returning to work, especially the assistive devices (AD) in workplace accommodation (WA). Currently, study related the assessment of the needs of assistive devices in WA after OA has been documented as not target to the context of the job, complicated and empirical. The International Classification of Functioning, Disability and Health (ICF) have been documented as a useful tool for assessing the AD. Though the complex of its context, it is important to find out the related factors of the needs of AD in WA for workers with OA.
Purpose: The purpose of this study is about to use the ICF for assessing workers with OA, and exploring their health condition and the needs of assisted devices, than using the classification and regression tree to explore the related factors of the needs of AD in WA to workers with OA.
Methods: This is a cross-sectional and exploratory study. The inclusion criteria for workers with OA is about the workers with OA in 5 years and qualification in medical or legal appraisal. The demographic information and ICF checklist were performed and recorded by evaluator. Evaluator determined the workers’ needs of AD in WA under the definition of WA, the duties and environment of work, and the function of the workers. The qualifies of the performance and ability to activities and participations in ICF was re-encoded for developing the discipline of the needs of AD in WA. Than we use the descriptive statistics was used to describe the personal information and the needs of AD in WA, the “classification and regression tree” and “logistic regression” to explore the related factors of the needs of AD in WA to workers with OA. The software for statistical analysis is SPSS 18.0 and S-PLUS 8.0.
Result: There are 142 workers in OA included in this study with average age 43.7±11.45, average years of the OA was 1.83±1.11, 59.9% male and 40.1% female. The return to work stage with the workers was major in vocational rehabilitation stage(79.3%), than follow by life rehabilitation stage(17.6%) and medical rehabilitation stage(2.8%).The most reason of accident were occupational injuries (92.3%), included car accident(38.1%), falling(19.7%), and so on. The most type of injury were fracture (45%), soft tissue injury(28.2%) , burn(15.5%) and spinal cord injury(14.8%). The most impaired body structure is neuromusculoskeletal and movement related foctors(b7), the activities and participation dimension with more problem is mobility(d4), self-care(d5) and domestic life(d6). The needs of the AD for the workers in OA is furnishings and adaptations to homes and other premises(52.1%), assistive products for personal care and protection(54.9%), assistive products for personal mobility(47.2%), orthoses and prostheses(46.5%), assistive products for handling objects and devices(44.4%), assistive products for personal medical treatment(31.7%) and assistive products for housekeeping(24.6%). The number of workers in OA who were determined as requiring of AD in WA was forty nine (35.5%).The related factors of the needs of AD in WA was “relating with strangers”(d730), “transferring oneself”(d420), “lifting and carrying objects”(d430), “fine and hand use”(d440), “walking”(d450), “driving”(d475), “drinking”(d560), “acquiring, keeping and terminating a job”(d845).
Conclusion: The way of assessment we use in this study can assess the health and functional status comprehensively for workers in OA. When combining the limit status of “activities and participation” and the discipline for AD and the definition of WA, we can comprehensive determine the need of AD for workers in OA. Though the characteristics for the participators in this study were specific in neuromusculoskeletal injuries, the related factors of the needs of AD in WA to workers with OA the we explore were explored by the study for return to work in the past, the related factors of the needs of AD in WA to workers with OA is about the workers’ interpersonal interactions and relationship, mobility, and self-care in workplace.
