According to the occupational fatality statistics by the Institute of Occupational Safety and Health of Taiwan, the significant number of occupational injuries as high as 162 in the formwork from 2000 to 2011. Formwork costly and requires highly specialized technology, in the event of an accident will result in casualties and properties damage, delays in the progress of works contracts may also be generated by the default of overdue fines. It has always been one big problem in the construction industry. In this study, in order to observe, expert interviews and questionnaire study was to investigate methods. Formwork engineering worked to identify hazards should be further analyzed to assess safety performance. The results show that and the main point should be noticed are: the relevance of statutes and impact of the company's staff number in safety management; the available labor and safety engineers, business owners' requirements, implementation procedure and other effects in the safety measures, the executive willingness of construction industry and the personnel selection in impact analysis in accident investigation.
