We analyzed the status, trend and associated factors of medical utilization in each district of Taichung City. By evaluating the adjustable factors, such as age, gender, illness and etc. in registered areas, we can offer some information for the policy makers in the hospital. Methods: We used the representative database of 1,000,000 subjects from the National Health Insurance Research Database of Taiwan of the year 2,000. The most visited sites of the treatment of upper respiratory tract infection and dental problems were assumed to be the virtual census register district. Totally there were 45,526 persons to be analyzed about the medical utilization in different districts. The statics included descriptive statistical analysis to describe the medical utilization in different district. T-test of independence, the one-factor analysis of variance and Sheffe test were used to analyze if there was significant difference in medical utilization for different personal qualities. Finally, we analyzed the associated factors by regression analysis. Results Results: All of the outpatient medical cost, inpatient medical cost and the frequency and cost of preventive health care increase year by year. From the viewpoint of gender, women are higher in outpatient medical utilization than men; while men used more in inpatient medical utilization. Speaking of age, people aged more than 65 used more in outpatient and inpatient medical utilization than any other age group. After adjusting other factors, the significant factors associated with outpatient and inpatient utilization are age, gender, preventive health care, chronic illness and major diseases. Factors significantly associated with total medical expense are age, chronic illness and major diseases. Conclusion: People with different personal qualities show significant difference in medical utilization. For outpatient medical utilization, women use more than men. While men use more than women in inpatient medical utilization. The elder, chronic illness suffer and being with major diseases also use more inpatient medical utilization. On the premise of free selection of enrollee in capitation pilot project, the age, with and without chronic illness and major diseases will be the major factors that the institute will take into consideration.
