This study used a single group pretest-post test experimental design that will meet the conditions of colorectal cancer patients, the study by researchers explain the purpose and the research process, to obtain 30 partients consent, given the pre-test, followed by "individualized discharge plan ", first of all, first giving the patient colorectal cancer related diseases knowledge, understanding and information needs of their self-efficacy, given after surgery care guide wound care, caregivers must learn to suffer wounds medicine techniques, which enhance the performance expectations and caregivers dressing results expected, before returning home caregivers need to reply to teach skills correctly wound dressing, wound care giving guidance related to health education leaflets given before returning home after the test, a week after returning home phone tracking patient; collected measurement data; collection procedures to close the case, the pre-test, individualized discharge care plan, such as post test stage 4. Results in SPSS For Window 20.0 software package for data archiving statistical analysis, colorectal cancer patients receive individualized care plans after discharge, process indicators have significantly improved patient quality of life, disease knowledge and self-efficacy improved significantly, p<0.0001 patient needs in the message, then there is a significant decrease, p value of 0.054, which means that there is a demand for the message to get satisfied. Caregivers, the caregivers perform wound care results and performance expectations, P value <0.0001 has significantly improved; patients and carers receiving individualized plan of care after discharge, p value of 0.001, for the satisfaction of health care has improved significantly. But in outcome indicators in length of stay, readmission rates and emergency was no significant difference, you may need more research to evidence. Currently colorectal cancer patients for individualized care plan is the lack of discharge can be used as reference and use relevant nursing research, this study individualized discharge care guide content, can be applied to clinical nurses in patient education on how through the use of teaching principles and materials tools, lead patients and caregivers effective learning.
