Backgroud: Incontinence-associated dermatitis (IAD) is a prevalent cause of skin damage in clinical settings. Damage to the skin may cause cauterization, pruritus and pain, and IAD is an increasingly prominent cause of secondary infections, which prolong hospitalization times and increase healthcare costs. Previous studies have proved that product could prevent IAD, but the evidence related to the skin barrier product with dimethicone is lacking. Methods: A quasi-experimental research design was used. A total of 100 adults who were fecal incontinence and APACHE≧15 from a medical center in Hualien. Subjects were randomly assigned to the experimental group (cavilon durable barrier cream, n = 50), the control group (ZnO, n=50). In the experimental group, skin care regimen was applied cavilon durable barrier cream twice a day/every time without/with stool passage. In the control group, skin care regimen was applied ZnO twice a day/every time without/with stool passage. The study was done in 7 days, and ended in occurrence of skin damage, discharged from ICU and expired. The outcome measures included the perineal assessment tool, occurrence of IAD and preesure ulcers, cost and efficacy of skin barrier product. The obtained data were analyzed using descriptive statistics, chi-square test, and generalized estimating equations. Results: Compared to the 2 regimen the perineal assessment tool scores and prevetion of IAD and pressure ulcers showed no significant statistically. Data for cost and efficacy, daily averge costs of study products were signifacantly higher in the experimental group than control group, daily averge costs of nursing time were signifacantly lower in the experimental group than control group, daily averge costs of medical consumption and total costs were no significantly difference. Conclusion: Based on research findings, cavilon durable barrier cream to reduce nursing time in clinical practice was recommended.
