The newly developed MR hepatocyte-specific contrast agent Gd-EOB-DTPA has different excretion pathways from convention MR contrast Gd-DTPA. There are two studies in my thesis; firstly, we compare the enhancement effect of liver and renal parenchyma between the two contrast agents in patients with liver cirrhosis. The other study is to compare the two contrast agents for hepatic tumors. The study included patients who underwent both Gd-DTPA and Gd-EOB-DTPA enhanced MRI within three months. A total of 49 patients were included for the study of liver and renal parenchyma. A total of 61 patients were included for the study of hepatic tumors. The enhancement ration (ER) at dynamic and delay phases were calculated by the same radiological technologist. The difference of the liver, kidney parenchyma and hepatic tumors between Gd-DTPA and Gd-EOB-DTPA were analyzed. The influence of liver function was also evaluated. The enhancement effect of liver parenchyma with Gd-EOB-DTPA was higher than with Gd-DTPA (p=0.004). Conversely, the enhancement effect of renal parenchyma with Gd-DTPA was significantly higher than with Gd-EOB-DTPA (p=0.01). The mean ERs of liver with Gd-EOB-DTPA for patients with normal serum bilirubin were significantly higher than with abnormal (p=0.047). The enhancement effects of hepatic tumors were similar between two contrasts, except for overt HCC at delay and hepatocyte phases. (p=0.002). In conclusion, the enhancement of liver and renal parenchyma by Gd-EOB-DTPA for patient with liver cirrhosis is different from Gd-DTPA.
