Description
Summary:博士 === 國立臺北護理健康大學 === 護理研究所 === 102 === The aim of this study was to investigate the results of solution-focused intervention in suicidal adolescents. This was a two-stage study. The purpose of stage one was to explore the lived experiences of the adolescent who has overcome his/her attempt of suicidal tendency. The purpose of stage two was to develop a solution-focused intervention based on the findings of stage one and evaluate its effects on overcoming suicidal thoughts in adolescents. Phenomenology, a qualitative research method, was applied in stage one. The participants were recruited with purposive sampling. A total of six teenagers aged 18 to 24 with a history of committing suicides participated in this study. They have not had suicide attempt in the latest one year. The study was carried out in a psychiatric out-patient service of a regional medical center in northern Taiwan. Semi-structured in-depth interviews were conducted to collect data. Next, content analysis was performed to analyze qualitative data. The axis of life experiences in suicidal teenagers identified in stage one was named “new start”. It comprised the following five major themes: conversion of suicidal thoughts, awareness of change, loving attachments, the emotional regulation, and the future prospects. Case study research was then carried out in stage two to investigate the effects of solution-focused intervention. The two cases of stage two were teenagers who have committed suicide in recent one month. Descriptive analysis was conducted to analyze data measured with quantitative method. The process and results of stage two are provided in the followings. First, the solution-focused intervention was designed with frames including “positive start”, “complains”, “goal” , ”presupposition”, “exception”, “consultation-breaking”, “feedback and task”, and “counseling-ending”. Each case accepted four to 4-6 sessions of interventions. The process of each intervention was taped and typed out word by word and analyzed with content analysis. The axis of qualitative analysis of outcome after intervention was “breaking waves”. The six concepts merged were “challenges from transition of carrier”, ”the review of scars of life”, ”the emotional regulation”, “the recognition of resources and strength”, ”the discovery of hope in life”, and ”the struggle between life and death”. The descriptive analysis of quantitative measurement: there was no significant change in case A’s BDI-II and Chinese version of The Positive and Negative Suicide Ideation Inventory (PANSI). However, case B had lower BDI-II and PANSI scores compared with pre-test. Both cases exhibited no suicide attempt in intervention and follow-up stages. The results of this study indicate that solution-focused intervention developed by the researcher could help teenagers to break through the impact from tide of life, find resources and strength, and start new life again with hope. Due to the limitation of small number of cases, statistic inferential was not recommended. The findings of this study could be applied to design programs or strategies preventing suicide in adolescents. Further application in practice and research was provided by the researcher.
