In order to comprehend the potential factors of causing industrial safety issues in the construction of factories and office building, and propose related suggestions, it’s expected to advance operation risk management and decrease the rate of onsite industrial accident as well. Among various the tools of risk identification in operation risk management integration tools(ORMIT) , in this research, the basic tools to analyze the risk source were applied. In total of 39 cases experimented through different methods, 25 cases based on preliminary hazard analysis(PHA) which is the most among all the methods used in this research; 5 cases of performing on what if tool(WIT); 4 cases of the scenario process tool(SP) and 5 cases of operation analysis(OA). 25 cases stay available after confirming and analyzing which approve the following experiments. The factors of causing industrial safety issues are observed and divided into five points of view in a basis of attributive proportion analysis, 22 items of personnel factor which takes 33.3% in all the points, 6 items of machine factor which takes 8.9%, 12 items of surrounding factor which takes 6.7%, 23 items of management factor which takes 35.6%, 16 items of task factor which takes 15.6%. To sum up, management element occupied the highest proportion among all the points; personnel factor comes second. It is able to sum up for following potential reasons that causes industrial safety issues, for example, insufficient professional knowledge of employed security guard, patrolling on a improper route, aerial working without certificate and lack of experience, no lying down plans on routine work and cleaning maintaining on mechanical firefighting devices on a year basis, misappropriating funds for entrusting fortune. The personnel who lack of skills and untrained with proper standards, is on a duty with improper attitude. Therefore, it comes up with three different points of suggestion to improve above mentioned situations. The suggestions based on the points of operation risk management:A.the point of supplier management :expanding the range of professional work in cooperation, abide by law and contracting out with strictness, respect the professional and ensure the supervision, raise the awareness of a crisis . B.the point of general contractor: provide service according to the contract content, advance the skilled assessment and the awareness of legality of managerial service personnel, ensure on site inspection and guidance, Attention on the salary and benefit of managerial service personnel. C.the point of operators: qualify legal employment and broaden related knowledge, choose the work condition with legality, follow standard operating procedure(SOP), ensure the operation risk assessment.
